Lead Home Care Nurse

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong understanding of medical documentation, anatomy, and various nursing practices, including home care and senior care. The Registered Nurse will work closely with patients, families, and other healthcare professionals to develop and implement individualized care plans.

Responsibilities

  • Conduct comprehensive patient assessments to determine health needs and develop appropriate care plans.
  • Administer medications and treatments as prescribed by physicians.
  • Perform medical procedures such as spinal taps, cardiac catheterization, and laboratory tests when necessary.
  • Maintain accurate medical documentation in compliance with regulatory standards.
  • Provide education and support to patients and their families regarding health management and home care practices.
  • Collaborate with interdisciplinary teams to ensure coordinated patient care.
  • Monitor patient progress and adjust care plans as needed based on clinical findings.
  • Assist in emergency situations by providing immediate care until further assistance is available.

Requirements

  • Valid Registered Nurse (RN) license in the state of practice.
  • Strong knowledge of anatomy, medical terminology, and nursing practices related to home health care.
  • Experience in senior care, clinic settings, or emergency room environments is preferred.
  • Familiarity with Minimum Data Set (MDS) assessments is a plus.
  • Excellent communication skills for effective interaction with patients, families, and healthcare teams.
  • Ability to work independently while demonstrating sound clinical judgment.

Job Types: Full-time, Part-time, Travel nursing, Contract, PRN, Permanent, Temporary

Pay: $45.39 - $75.95 per hour

Expected hours: 8 – 40 per week
