Home Health Nursing Manager

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that promote optimal health outcomes.

Duties

  • Conduct comprehensive assessments of patients' health status and needs in their home environment.
  • Develop, implement, and evaluate personalized care plans based on patient assessments.
  • Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
  • Provide education to patients and families regarding health management, medication administration, and care procedures.
  • Monitor patient progress and adjust care plans as necessary in collaboration with the healthcare team.
  • Supervise home health aides and support staff in delivering quality care.
  • Maintain accurate documentation of patient care activities, assessments, and outcomes.
  • Collaborate with social workers to address social determinants of health affecting patient well-being.
  • Utilize financial acumen to assist patients with navigating healthcare resources and budgeting for care needs.

Requirements

  • Current Registered Nurse (RN) license in the state of practice.
  • Experience in home community care or long-term care settings such as nursing homes or assisted living facilities is preferred.
  • Strong knowledge of medication administration practices and safety protocols.
  • Excellent interpersonal skills with the ability to communicate effectively with patients, families, and interdisciplinary teams.
  • Familiarity with social work principles to support holistic patient care.
  • Ability to supervise support staff while fostering a collaborative team environment.
  • Strong organizational skills with attention to detail for accurate documentation and reporting.
  • Commitment to providing compassionate care that respects the dignity of each patient.

Pay: From $83,647.00 per year
