We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that promote optimal health outcomes.
Duties
- Conduct comprehensive assessments of patients' health status and needs in their home environment.
- Develop, implement, and evaluate personalized care plans based on patient assessments.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Provide education to patients and families regarding health management, medication administration, and care procedures.
- Monitor patient progress and adjust care plans as necessary in collaboration with the healthcare team.
- Supervise home health aides and support staff in delivering quality care.
- Maintain accurate documentation of patient care activities, assessments, and outcomes.
- Collaborate with social workers to address social determinants of health affecting patient well-being.
- Utilize financial acumen to assist patients with navigating healthcare resources and budgeting for care needs.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home community care or long-term care settings such as nursing homes or assisted living facilities is preferred.
- Strong knowledge of medication administration practices and safety protocols.
- Excellent interpersonal skills with the ability to communicate effectively with patients, families, and interdisciplinary teams.
- Familiarity with social work principles to support holistic patient care.
- Ability to supervise support staff while fostering a collaborative team environment.
- Strong organizational skills with attention to detail for accurate documentation and reporting.
