Medical Director /Primary Care Provider

ATN Healthcare is home care agency and a virtual care platform to integrate primary care, home care, home health, HCBS, specialty care, mental health, dietician, and health coach from multiple locations to one virtual place when needed. Our Medical Director and Primary Care Provider is an experienced family physician, who values listening, learning, teaching and team-building. We aim to deliver better patient-centered comprehensive coordinated care across specialties and provider types.

How will my time be spent?

Patient Visits
Extended in-person and/or virtual intake visit, with subsequent visits every 3-6 weeks/months depending on patient acuity
Asynchronous collaboration with the same PCP for most of your patients, who will coordinate preventive care, mental health medication management, and non-rheumatology related concerns
PCP will also help coordinate care with the health coach, mental health provider, and specialty team, so your patients are hearing advice from one, coordinated team
Use our streamlined EMR to document patient-facing assessments and plans, with an emphasis on goal setting and forward progress for whole body health
Home call one night/weekend per week for critical lab and imaging results
Help design an after hours triage protocol to support patients with more urgent needs
Teach our patients and our community
Help make blog posts, videos, and/or patient handout creation
Using AI, help us learn which interventions work best and for which patients
Help our great EMR be even better
Help provide feedback on our home-grown EMR so that we can make it better in real time
Maintain a birds-eye view of our mission to help our processes, team, and infrastructure evolve to best suit our patient and provider needs.
You’ll be a strong applicant if you:

Have at least 1 years of experience in rheumatology
Are a graduate of state-accredited MD or DO residency program
Have at least one active state medical license
Use an evidence-based approach to caring for patients, and remain open to learning more
Are eligible for a medical license in Colorado. Our team will support you to secure license, registration, and credentialing in necessary states.
Have experience working effectively on teams to care for patients
Have excellent written and verbal communication skills
Have the ability to thrive in a fast-moving and constantly evolving, start-up environment. The candidate will deliver technology enabled, data-informed, compassionate patient care.
